Inspection observation record

Record a site observation with time, location, context and the required follow-up.

The observation record separates what was directly seen from what was reported by others. It supports a scoped inspection without implying a whole-site compliance verdict.

Worked example

The example records an obstructed route at a particular location and time. It identifies an immediate communication and leaves the broader cause and follow-up evidence unresolved. A later clear route should be recorded as a later observation, not used to erase the original.

Use the resource

Use the project’s agreed urgent-response route where necessary. Obtain appropriate permission for photographs and protect identifying information. The worksheet is a record structure, not operational safety instructions.
