Evacuation exercise debrief record

Capture exercise observations, participant feedback and actions for a competent debrief.

The debrief records what the exercise was intended to test, what was observed and what needs follow-up. It does not certify an emergency plan.

Worked example

The illustrative row records that a participant did not hear an instruction during the exercise. It asks which location and communication conditions applied. It avoids assigning a cause without evidence or using an invented universal evacuation-time target.

Use the resource

Agree the exercise scope, observer roles and safety arrangements through the organisation’s competent planning process. Keep real emergency response distinct from a planned exercise. Record follow-up owners and how changes will be reviewed.
