Worked example
The illustrative issue concerns a recurring access obstruction. The representative records the location and communication, while the responsible manager’s action and follow-up evidence remain separate. Reporting an issue does not demonstrate that it has been resolved.
Use the resource
Use neutral factual wording and protect personal information. If the issue requires immediate action, use the workplace’s agreed route. Keep routine meeting feedback connected to the original issue reference.
