Follow one fictional delivery task from hazard to a control question and verification record.

This worked example demonstrates how to keep a task, hazard, possible consequence, existing arrangement and follow-up question connected. It is not a site risk assessment or an instruction to use these controls.

Worked example

Illustrative task: deliveries enter an area also used by pedestrians. The example identifies the vehicle/pedestrian interface and a possible struck-by consequence. It records that the current separation arrangement needs verification. It deliberately avoids assigning invented numerical risk ratings or declaring the activity safe.

Use the resource

Use your organisation’s approved assessment method and competent participants for real work. Replace the fictional context, establish the actual conditions and evidence, and agree who can decide and review controls. Keep a record of assumptions and information that remains unavailable.
