Design-stage risk discussion register

Record design-stage H&S issues for the person authorised to decide the design response.

This register records an issue, its drawing or activity reference, the design decision owner and the outcome. It does not authorise a design change.

Worked example

The illustrative row asks about access for a future maintenance activity. The design authority determines the response using the relevant information; the register records that decision and any information to pass to later stages.

Use the resource

Keep the drawing revision visible and update the link when the design changes. Record unresolved questions and the next review point. Do not mark the issue closed solely because it was discussed in a meeting.
