The worksheet keeps identity, category or scope, source, verification date and unresolved conditions together. A logo or copied certificate image is a starting point for checking, not the result.
Worked example
The example asks whether a proposed practitioner’s name, number and category match the relevant professional-body record. For a course claim, use the applicable issuing or quality-assurance body and verify the exact programme and provider relationship.
Use the resource
Record the evidence you actually obtain. If an official record is unavailable or ambiguous, mark the result unresolved and request clarification. Do not describe an attempted search as successful verification.
