Corrective-action tracker

Track findings through ownership, completion evidence and a separate closure decision.

The tracker keeps implementation and verification in separate fields so an owner’s completion message is not silently treated as verified closure.

Worked example

The fictional row follows a recurring obstruction observation. Its status remains open until the agreed action and evidence have been reviewed. Replace the example with the actual finding, criteria and response process.

Use the resource

Use dates agreed by responsible people; the worksheet does not invent priority deadlines. Record the reason for overdue or reopened items and the escalation route. Preserve the original finding when responses change.
