The comments log preserves the original question while tracking each response and the reviewer’s decision. Acknowledgement is a separate state from resolution.
Worked example
The example asks which attachment version belongs to the current submission. The response “noted” is recorded but does not close the comment. Closure needs a clarified issue set or other relevant evidence accepted through the agreed review process.
Use the resource
Assign a stable comment reference and retain it across revisions. Keep the plan version and section with the question. If a response creates another question, record that relationship rather than replacing the original history.
