Audit evidence index

Index evidence against the agreed audit criteria, with versions, owners and missing-item status.

An evidence index tells a reviewer where the relevant record is and whether it is current and available. It does not decide whether the record satisfies the criterion.

Worked example

The example links an action-closure criterion to the current action register and a supporting folder. It marks verification evidence as missing, keeping that gap distinct from the register’s own status label.
